Perth Diabetes Care now has a Teen and Young Adult Clinic for those living with Type 1 and Type 2 Diabetes.
What do we offer?
Perth Diabetes Care currently offers the following services as part of our Teen and Young Adult Diabetes Clinic:
- Diabetes Educators
- Exercise Physiologist
We currently do not have a private endocrinologist at the clinic but aim to include this service in the future.
How to Refer?
- You can self-refer although full fees will apply.
- Your GP can also refer you to the clinic and if they complete a GP management plan (GPMP) and a Team Care Arrangement (TCA) you may be eligible for a rebate from Medicare or one of the insulin support programs.
- If you are currently seeing a private endocrinologist they may also provide a referral for allied health support at the clinic.
If you are transitioning from a public service diabetes clinic you will still require an endocrinologist. We can work with your endocrinologist to support you at the Teen and Young Adult Diabetes Clinic provided by Perth Diabetes Care.
